The Registrar's Office issues and maintains birth and death certificates for unincorporated communities only. Villages have their own clerk and vital record offices. Facilities under Hempstead Town's jurisdiction are:
- Nassau University Medical Center in East Meadow
- South Nassau Communities Hospital in Oceanside
- Franklin Hospital Medical Center in North Valley Stream
There is a $10.00 fee per certified copy of a birth or death certificate requested. Payment may be made by cash, local check, money order or credit card (a $6 VitalChek service fee applies for all credit card transactions). Please make checks and money orders payable to Nasrin Ahmad, Town Clerk. Pending approval of an application, a request will be processed and mailed within one to two business days. Expedited processing is available using a major credit card. The total fee for expedited processing is $33.50, which includes one certified copy of the certificate, UPS shipping and a VitalChek service fee.
Lost Records Due To Sandy
Now extended through December 31, 2015, the Hempstead Town Clerk’s Office is waiving fees for replacement birth, death and marriage certificates, as well as marriage transcripts lost in Superstorm Sandy. Residents must show a FEMA claim or homeowner’s/flood insurance claim in order to take advantage of the fee waiver. Nassau County residents who have lost records in the storm (deed and mortgage documents) can call the Nassau County Clerk at (516) 571-6377 to inquire about replacement records (New York State fees apply).
