The Registrar's Office issues and maintains birth and death certificates for unincorporated communities only. Facilities under its jurisdiction are the Nassau University Medical Center in East Meadow, South Nassau Communities Hospital in Oceanside and Franklin Hospital Medical Center in North Valley Stream. Villages have their own clerk and vital record offices.

There is a $10.00 fee per certified copy requested. Payment may be made in cash or by local check or money order made payable to Nasrin Ahmad, Town Clerk. Credit Card service is available to expedite requests. Office hours are Monday, Tuesday, Wednesday and Friday, 8 AM to 4:45 PM, and Thursday, from 8 AM to 7 PM.  

Lost Records Due To Sandy

Now extended through September 30, 2015, the Hempstead Town Clerk’s Office is waiving fees for replacement birth, death and marriage certificates, as well as marriage transcripts lost in Superstorm Sandy. Residents must show a FEMA claim or homeowner’s/flood insurance claim in order to take advantage of the fee waiver. Nassau County residents who have lost records in the storm (deed and mortgage documents) can call the Nassau County Clerk at (516) 571-6377 to inquire about replacement records (New York State fees apply).
